SECTION E : TERMS AND CONDITIONS

I/we understand that completing this form (the "Waitlist Form") does not guarantee the acceptance of my child(ren) to Al-Falah Islamic School ("AFIS").

I/we understand that my submission of the Waitlist Form will be processed sequentially in accordance with the following waitlist policy (the "Waitlist").

The Waitlist operates with the following priority structure:

a. Staff child
b. Sibling of an existing student (the "Sibling Priority List")
c. Re-enrollment of a former student
e. New student

I/we understand that once a spot becomes available at AFIS, I/we will be notified and agree to the following process:

a. AFIS will call the Parent(s) or Guardian, and if the call is not answered, a voicemail will be left followed by an email notification.
b. I/we will respond within 72 hours to confirm my interest. Failure to comply will result in the removal of my child(s) application from the Waitlist.
c. I/we acknowledge that a pre-assessment interview or entrance test will be scheduled by AFIS within five business days for my child(ren).
d. I/we acknowledge that my child(ren) will be required to appear in person for a written entrance test (Grades 1-8) and (or) a pre-assessment interview (Montessori).
e. I/we acknowledge that my child(ren) will be required to appear in person for an interview with AFIS Administration and (or) Teacher(s).
f. I/we acknowledge that I/we as the Parent and (or) Guardian of the child(ren) will be required to appear in person for an interview with the AFIS Administration.
g. For Montessori specifically, I/we acknowledge and ensure compliance with the Montessori Readiness List before the interview.
h. I/we acknowledge that my child must pass the written entrance test (Grades 1-8: 75% or more on each assessment) and interviews with the Administration and (or) Teacher before my child(ren)'s admission to AFIS.
i. I/we acknowledge that if my child fails the written entrance test or interview, he/she will be removed from the waitlist without a refund of the Waitlist Fee.
j. I/we acknowledge that in the scenario where a spot is made available for a sibling, the other sibling(s) will be placed on the Sibling Priority List and that there is no guarantee that the sibling(s) will be admitted into AFIS at the same time.
k. I/we acknowledge that if my child(ren) is offered a spot, I/we will complete the online Admission Form within 72 hours. Failure to comply will result in the removal of my child(s) application from the Waitlist.
l. I/we acknowledge that my child(ren) will only be registered into the respective grade based on their birthdate, as per the Ontario Ministry of Education cutoff date.
